If your insurance provider is not listed
on this page, payment is due at the time the services are rendered, and you will be responsible for filing with your insurance
company.

Please be advised that your doctor may order
tests or blood work that are not covered by your insurance company. It is your responsibility to know your insurance.
Should a test require precertification, we will do all that is necessary to provide the proper information to the insurance
company in order to obtain approval. In addition,
we will do our best to obtain prior authorization for medications that your doctor prescribes, if your insurance requires
it in order for you to obtain the medication. However, due to the excessive amount of time required to handle
the number of patients requiring this service, we cannot do medical necessity letters or make prior authorization calls
to your insurance company to lower copayments. |
